Registration/licensing application forms

This is a reminder that we will no longer accept applications for registration or licensing on the old application forms. Our new licensing and registration requirements have been operational since July 1999, and are on the website. The new forms are provided at the back of each package and they can also be found listed under ‘Forms’. Should we receive an application on the old formatted version, you will be contacted and asked to resubmit it, using the correct forms.

Details on forms
In most cases, the new application forms are being filled in correctly. However, in some applications for a ‘variation of registration’, it has been noted that the concentration details given in the formulation differ from those in the original application. Provided there has been no change in formulation (in which case we should be advised), the application forms should be of the same accuracy as previously supplied. Further to this, concentration details contained in the formulation should be given in the correct units, i.e. %w/v or %w/w.
