Adverse Event Reporting - Change Of Address |
The timely reporting of adverse events by all users of animal remedies is the most important first step in enabling licensees, manufacturers and the regulatory authority to implement remedial actions that will prevent continued use with possible distress to animals. We are aware that some people are still using Adverse Event Report forms that were issued prior to the move of the ACVM Group from Wallaceville to Wellington. In addition, the form included in the current IVS carries the old address. When reporting an adverse event please would you send or fax the form to the following
