Animal Status Declaration (ASD)

The ASD form is a legally mandated form authorized by the NZFSA and the Animal Health Board. The ASD covers cattle, deer, sheep, goats, horses, alpacas, llamas, ostriches and emus. A separate form covers pigs - see below.

An ASD must be completed for all animals sent for processing. It must be completed for all animals (except alpacas, llamas and horses) moved from one property or salesyard to another property or salesyard or property where there is a different person in charge. The ASD is to be completed by the person in charge of the animals.

NB: AHB and MINDA numbers (Needed for cattle and deer only) are provided by the Animal Health Board - phone 0800 482 4636.

(It must be printed exactly (no additions, no subtractions) as published on the NZFSA website. This means it should be at 100% of the A4 size.)
