Animal Status Declaration (ASD)
The ASD form is a legally mandated form authorized by the NZFSA and the Animal Health Board. The ASD covers cattle, deer, sheep, goats, horses, alpacas, llamas, ostriches and emus. A separate form covers pigs - see below.
An ASD must be completed for all animals sent for processing. It must be completed for all animals (except alpacas, llamas and horses) moved from one property or salesyard to another property or salesyard or property where there is a different person in charge. The ASD is to be completed by the person in charge of the animals.
NB: Every person owning cattle or deer must register those animals as a herd with the Animal Health Board (0800 482 4636). Livestock Improvement (LIC) provides MINDA registration which is optional.
It MUST be printed EXACTLY as published on the NZFSA website. This means it should be at 100% of the A4 size.
