Application Form for Access to Animal Products E-cert

Instructions

Please note: You cannot apply for access to E-cert unless your organisation has completed the 'E-cert Organisation to be Billed Form'. This can be found at: http://www.nzfsa.govt.nz/ecert/animal-products/billing/index.htm

  1. Fill out the form below.
  2. After you press the submit button at the bottom of this page a new page will show on screen. This new page will contain the details you have entered here.
  3. Print the new page.
  4. Get the appropriate signatures and send the completed form to NZFSA.
  5. Your application cannot be processed until we have a signed document including a billing organisation from you.

E-cert operates under a user pays cost recovery regime. The "Organisation to Bill" is the name of the organisation that will be billed for your use of E-cert. If you require clarification about what organisation you should name, please contact your company E-cert Coordinator.

Premises or
Exporter ID(s):

List the premises/exporter (s) IDs (eg ME22, S181,EXPORTER1) for the E-certs that you process.
Role(s): Consignor
The consignor can submit certificates for  products which leave  the consignor premises.
Consignee
The consignee can view certificates for products which arrive at the consignee premises. If you are a consignor you should also be a consignee.
Auto Approval Auto-approval allows the computer to auto approve Eligibility Documents for those people authorised to use this function. If you are authorised to action Auto-Approval as well as action EDs with MAF VA, you must have a separate E-cert User ID for each action. Therefore you will need a separate E-cert ID and login for Auto-Approval and one for those which require MAF VA confirmation.

At this time the only people to use this function are:

  Seafood Users. As outlined in GREX 03/021. To be eligible for seafood auto-approval you must either be a current seafood signatory or have your local MAF Verification Agency Team Leader countersign the form and they must also state on the form that the person should have auto approval permissions and specify which premises.

  Petfood UsersPetfood Users for the Domestic Market. The operator must additionally have the form signed by their local MAF Verification Agency Team Leader who must also state on the form that the person should have auto approval permissions and specify which premises. E-cert export market petfood users do not have access to the auto approval option.
