Form 15.1: Recall Hazard/Risk Analysis Worksheet

This form is used by NZFSA and Food Act Officers (at your local Public Health Unit) to assist with making recall decisions. Manufacturers may wish to incorporate use of this form into a recall plan, or in the event of a recall can use this form as a guide for preparing information to be provided to a Food Act Officer or NZFSA.
